Best Care Program
The Best Care Program is designed to support patients living with congestive heart failure (CHF), right here in our community.
Through this program, a specialized Case Manager is embedded in our clinic to work alongside your primary care provider.
Together, they offer:
- Personalized assistance with diagnosing heart failure
- Treatment planning and optimization
- Medication management
- One-on-one patient education and support
This collaborative, team-based approach ensures you receive high-quality, individualized care close to home, with the tools and knowledge you need to feel confident managing your condition.
Geriatric Program
Cognitive Screening
Our goal is to gather the essential data needed to aid the primary care provider in a potential diagnosis of Dementia. Early intervention and personalized care plans are an important step towards managing cognitive health.
Fall Prevention
Falls are the leading cause of fatal and nonfatal injuries among adults 65 and older. Depending on the severity, falls can lead to pain, loss of movement and function, and even hospitalization. Our goal is to work with seniors to help maintain their independence and reduce their risk of having a fall.
During the 30-minute appointment, a Registered Nurse will meet with patients and provide the following:
- Falls risk assessment
- Education on ways to reduce the risk of falling
- A take-home checklist for self evaluation
- Connection to community programs and services

Hypertension Management
The Hypertension Management program helps patients prevent and manage high blood pressure. A Registered Nurse reviews their history and health risk factors. Together with the nurse each patient develops a lifestyle based plan.
Services provided:
- Health assessment ( blood pressure checks and advanced monitoring)
- Information and health education
- Support and counselling

Mental Health Therapy
Our experienced mental health therapist offers short term counselling for patients of all ages with mild to moderate symptoms to address such issues as:
- Anxiety
- Depression
- Grief
- Relationships
- Stress

Nurse Practitioner Primary Care
Nurse Practitioners assess, diagnose, treat and monitor a wide range of health problems using an evidence based approach to their practice. They consult and collaborate with other health care professionals to meet the needs of the patient population. Chronic disease management, health promotion and illness prevention, as well as helping patients navigate through the health care system are integral to their approach to patient care. Engaging patients as full partners in their care plan with attention to self-care to the extent that patients are willing and able to participate is an important aspect.

Working together with patients, primary care providers, and specialists, we can provide more care options and innovative solutions that help improve access to specialized care; reduce pressure on hospitals; and modernize consumer access to care.

Smoking Treatment for Ontario Patients (STOP)
Ready to quit smoking? The STOP Program offers free nicotine replacement therapy (like patches or gum), one-on-one support, and personalized strategies to help you succeed.
We know quitting isn’t easy — but with the right tools and support, it’s possible. This program is confidential and open to anyone ready to take the first step toward a smoke-free life.

Therapeutic Nutrition Counselling
New to AHFHT, this service includes one-on-one therapeutic nutrition counselling. Patients are encouraged to speak to their Primary Care Provider for a referral to the Dietitian.
Type II Diabetes Management
The program includes education and support with:
- Nutrition and meal planning
- Goal setting
- Medications
- Foot care
Well Baby Program
Our Well Baby Program staff routinely assess a child’s growth and development and support parents and caregivers by answering questions, providing education, and linking with community resources.
Well Women Program
Our Well Women Program staff provide education to increase awareness of women’s health issues, including routine cancer screening, pessary care, menopause, STI screening/testing and overall contraception counselling.
