Find a Primary Care Provider
We serve residents of Burk’s Falls, Ryerson, Armour, Perry, Magnetawan, Kearney and McMurrich-Monteith.
We are working with Health Care Connect to attach you to a Physician or Nurse Practioner as soon as one becomes available. If you live in the Burk’s Falls catchment area, (see map) and you don’t have a family doctor or nurse practitioner, we can help you find a physician or nurse practitioner in your community.
Register yourself and/or your whole family for Health Care Connect and a care connector will search for a doctor or nurse practitioner who is accepting new patients in your community. Patients currently on our internal AHFHT waitlist are free to join Health Care Connect if they wish. This will not affect their status on our internal list.

Monday to Friday, between 8:30 a.m. and 4:30 p.m.
