About AHFHT
Almaguin Highlands Family Health Team is a group of interdisciplinary health care providers and administrators providing programs and support to the patients who receive primary care from our affiliated physicians.
Our team provides primary care services based on our local health and community needs, with a focus on health promotion, disease prevention, chronic disease management and mental health.


About Family Health Teams
A Family Health Team (FHT) is a primary healthcare organization in Ontario that brings together different healthcare providers to offer comprehensive care to patients. Family Health Teams typically include physicians, nurses, nurse practitioners, social workers, dietitians, and other healthcare professionals.
The goal of FHTs is to provide patients with access to a wide range of services in one location, promoting continuity of care and improving health outcomes. Many teams focus on preventive care, chronic disease management, and coordinating services for patients with complex medical needs.
Ontario Family Health Teams were established in 2005. Currently there are 184 Family Health Teams in over 200 communities across the province, serving over 3 million people.

In compliance with the Accessibility for Ontarians with Disabilities Act, 2005, Almaguin Highlands Family Health Team (AHFHT) is committed to giving people with disabilities the same opportunity to benefit from the same services and opportunities, in the same place and in a similar way to others.Communication
We will communicate with people with disabilities in ways that consider their disability.
Service animals
Service animals for individuals requiring help to function with greater self-sufficiency; prevent injuries; and summon help in a crisis are welcome on these premises.
Support persons
A person with a disability who is accompanied by a support person will be allowed to have that person accompany them on our premises.
Notice of temporary disruption
In the event of a planned or unexpected disruption to services or facilities for patients with disabilities, AHFHT will notify patients promptly by clearly posting notice including information about the reason for the disruption, its anticipated length of time, and a description of alternative facilities or services, if available on the doors of the clinic and if possible, on AHFHT’s website.
Feedback process
Patients who wish to provide feedback on the way AHFHT provides goods and services to people with disabilities can provide feedback by:
- Verbally speaking to a staff member, or
- Via email at feedback@ahfht.ca, or
- By telephone: 705-382-4020, or
- In writing or letter: Addressed to Almaguin Highlands Family Health Team, PO Box 550, Burk’s Falls, ON P0A 1C0
The feedback provided will be investigated by AHFHT management team and findings will be communicated back to the patient within two (2) weeks, unless otherwise notified, i.e.: timelines may be impacted by staff vacations, statutory holidays, etc.
